Forms Marriott Electrical and Audio Visual Equipment Form - If you need electricity, download the form and follow the instructions. – Orders placed by Wednesday, 8/18/10 will receive a discounted rate. Temporary Food Permit Instructions Montgomery County Health Permit - A temporary health permit must be obtained by mail two (2) weeks in advance of the event date (Prior to Friday, 08/13/10) or in person two (2) full business days before the event date (Prior to Wednesday, 08/25/10). A late fee of $25.00 will apply if not obtained in advance. |
